CPC Community Health currently has two innovative programs that reach into communities to help people find effective ways to become, active, empowered and healthy.
Community Heart Health Actions for Senior Latinos at Risk (CHARLAR) is a 12 week heart disease and diabetes prevention program tailored specifically for older adult Latinos (50+ years) residing in Northwest Denver. The program uses an evidence-based education curriculum based on the Pasos Adelante (Steps Forward) program that emphasizes risk factor awareness, diabetes education, and skill building for healthy eating and physical activity.
Colorado Heart Healthy Solutions (CHHS) is a statewide program that involves a network of community health workers (CHWs) who reach out to underserved populations and provide health assessments, health education, referrals to local medical and healthy living resources, and ongoing support to at-risk participants via case management at no cost.
