Outreach, Screening and Referral (OSCAR) System
OSCAR is a public health solution developed by CPC Community Health. It is an easy-to-use dashboard software system that provides community health workers with the evidence-based clinical support they need to improve the health of individuals in their community.
How the OSCAR system supports our programs
Empower CHW Workforce to:

Provide individualized health recommendations via evidence-based decision support

Identify trends in health factors (such as blood pressure levels) and health behaviors (such as nutrition and physical activity)

Manage health goals of participants

OSCAR and our programs are available for license from CPC Community Health. We provide instructor manuals with the curriculum, our Outreach, Screening and Referral (OSCAR) system, initial training and setup, and ongoing support.
